The success of every student in our clinical learning environment (CLE) is a top priority for Prisma Health. As such, the purpose of this form is to provide a space for students, academic institutions or Prisma Health team members to submit concerns regarding the safety of the CLE (any issues related to PPE, harassment, patient care, etc.) for the purpose of intervention. The goal is to ensure that we hear from all parties when there is a concern and that, most importantly, we are able to provide a safe environment for our students, team members, and patients.
